Bilingual Haitian Creole or Spanish speaking LPN
Job Location: Midtown Manhattan, NY; 4 days in office, 1 day remote, with occasional travel to clinical sites in NY & NJ areas.
Job Schedule: Mon-Fri, 9am-5pm.
Job Type: Full Time.
Pay Rate: $60,000-$70,000 per annum ($30-$33/hr).

Job Description:
VitalCheck Wellness is seeking a fulltime Licensed Practical Nurse (LPN) for our Diabetes and Chronic Disease Management Program, to coordinate the program, ensure patient adherence and provide education to employees of our clients in the NY and NJ area. We are looking for someone passionate about improving patients' lives and delivering high-level customer service.

Responsibilities:

  • Coordinate between patient, clinical, and operations partners to set up appointments, follow up, and get feedback.
  • Ensure a high adherence rate to the program in accordance with our guidelines.
  • Provide education around diabetes management and exercise advice to help clients establish healthy eating habits and lifestyle.
  • Support clients in diabetes management and other aspects of well-being and lifestyle.
  • Establish and maintain rapport and good relationships with patients.
  • Maintain client documentation.

Requirements:

  • Bachelor’s degree in nursing, health administration, or a related field.
  • Active license of Licensed Practical Nurse (LPN) in NY or NJ strongly preferred.
  • Three to five years of experience in nursing, health administration, or a related field.
  • Experience with educating on diabetes or other chronic conditions.
  • Experience with vaccine administration and phlebotomy strongly preferred.
  • Proficient with computers, Microsoft Office, and EMR systems.
  • Customer service-oriented with great bedside manner.
  • Strong communication skills; both written and verbal.
  • Ability to work independently with little supervision.
  • Bilingual in Haitian Creole or Spanish strongly preferred.
  • Must be able to reliably commute throughout NY and/or NJ.
