Job summary

Traditional Home Care (Habilitation Division) in State College, PA seeks a Habilitation Manager to oversee caseload, staff, and deliver direct services to individuals with intellectual and developmental disabilities. You will promote independence, community participation, and job finding while ensuring compliant case management.

The ideal candidate is a self‑starter, comfortable working independently, and able to juggle multiple tasks with strong communication and organizational skills.

Qualifications

  • High school diploma required; Bachelor’s degree preferred.
  • Experience with IDD services and ODP/agency requirements is desirable.
  • Strong communication and organizational skills; ability to travel.

Responsibilities

  • Oversee assigned caseload and staff; ensure quality services.
  • Coordinate scheduling, recruitment, hiring, and supervision of staff.
  • Maintain compliant case management and updated records.
  • Build positive relationships with clients and families; report changes.
  • Attend meetings (ISP, intake) and document paperwork accurately.
  • Promote client independence and community participation.

Job description

Traditional Home Care (Habilitation Division) has an immediate opportunity for a Habilitation Manager in our State College, PA office.

The Habilitation Manager will oversee assigned caseload, staff, and provide direct services to individuals with intellectual and developmental disabilities. This position will help people who live at home by promoting independence, working on personal goals, helping the individual participate in community activities, assist in job finding, and various other daily tasks.

The ideal candidate will be a self-starter who is comfortable working independently and is able to handle multiple tasks and assignments simultaneously.

Duties and responsibilities include, but not limited to the following:
  • Manage assigned caseload of clients and staffing cases appropriately.
  • Obtain training/certification according to ODP and agency requirements and Inclusive Academy training programs.
  • Leading assigned caseload, including assisting in scheduling, recruiting, hiring, and supervising assigned caseload staff.
  • Maintain positive relationships with clients including informing clients of agency activities, notifying clients of schedule and staff changes.
  • Conduct and document monthly ‘touch base’ meetings with clients / families, including service satisfaction reviews, and other pertinent information.
  • Ensure compliant case management for ODP systems, including maintaining updated files and other compliance matters as directed.
  • Attend agency meetings as requested including ISP, Intake meetings, and others as necessary and file all paperwork properly as required.
  • Other related duties as assigned.
Requirements / Qualifications
  • High school diploma required; Bachelor’s degree preferred
  • Positive, enthusiastic attitude and professional appearance
  • Office of Developmental Programs experience - 2 years preferred
  • Prior experience in home care scheduling, recruiting or management experience preferred
  • Experience in ID compliance, case management, HCSIS, staff supervision, and activity planning preferred
  • Excellent communication, presentation, and social management skills
  • Computer knowledge, strong documentation and organizational skills.
  • Valid PA Driver's License, clean driving record, and willing to travel
  • Ability to work independently without constant supervision
