Job summary

Worland Health and Rehabilitation in Worland, WY is seeking a licensed Physical Therapist to join our fast-paced team. You will assess residents, develop treatment plans, and collaborate with the Rehabilitation Manager to deliver evidence-based care.

The role emphasizes compliance with state practice acts and APTA guidelines, documenting care, and participating in discharge planning and quality improvement initiatives. Training opportunities available.

Qualifications

• Bachelor, Master of Science, or Doctorate in Physical Therapy from an accredited program.
• Successful completion of National Certification Exam for PT.
• Current licensure in the state of practice where applicable.

Responsibilities

• Comply with state practice act and/or American Physical Therapy Association (APTA) guidelines.
• Develop and implement treatment plans with physician order; monitor and adjust as needed.
• Participate in continuous quality improvement activities.
• Identify and advise Rehabilitation Manager of issues needing attention.
• Communicate resident/staff issues affecting the caseload.
• Develop/implement Physical Therapy services with RM and Center staff.
• Participate in discharge planning.
• Supervise PT assistants and aides where applicable.
• Participate in RAFT activities to identify residents in need of rehab services.
• Provide PT screenings and evaluations to determine functional level before treatment.
• Maintain productivity level as determined by the RM.
• Self-monitor productivity daily.
• Participate in resident/family and Center meetings when appropriate.
• Follow administrative policies related to emergencies, infection control, and safety.
• Maintain consistent and predictable hours of service in buildings as assigned.
• Provide clear documentation in medical records related to reimbursement guidelines.
• Document and sign each treatment encounter in medical record.
• Complete progress notes per Center policy.
• Participate in Quality Assurance Review and denials process as requested by Rehab Manager.

Education

Bachelor/Master/Doctorate in Physical Therapy (ACGME-accredited program)
