Description
Scope: Full Time clinical position responsible for facilitating Residential Services groups (3/4 of time worked) and providing ongoing case management and discharge planning (1/4 of time worked).
Duties:
- Conduct Residential Services groups and document in EHR. Develop and utilize curriculum and community resources to plan and facilitate (or co-facilitate) assigned groups. May facilitate discharge planning groups.
- Communicate with other clinical staff any client issues that arise in group sessions.
- Participates in weekly treatment team meetings and provides pertinent updates on client progress.
- Provides a Discharge Planning Needs Assessment for all residential clients within 1 week of admission.
- Coordinates discharge planning for clients who are referred by psychiatrist. Meets with clients to discuss and develop individualized discharge plan. Works with primary counselor to identify needs for outpatient services and identifies options for outpatient providers based on psychiatrist discharge order. Has knowledge of clients course of treatment and goals. Compiles all clinically significant information relevant for developing and finalizing the discharge plan/instructions.
- Completes discharge documentation as ordered by psychiatrist: provides blank discharge order to psychiatrist during weekly treatment team meetings; completes discharge instructions on the day of discharge, provides to clients (and family members when appropriate); ensures completion NIS form prior to discharge (when appropriate); notify staff of transfers/ discharges in Profiler and via email. Completes discharge packet for clients.
- Coordinates logistics for voluntary and involuntary psychiatric hospitalizations of current clients. Contacts local hospitals, providing clinical information and insurance information to secure a hospital bed for client in crisis. Arranges transportation to hospital. Keeps clinical team apprised of situation.
- Ensures each assigned clients level of care is up to date in Electronic Health Record.
- Documents communication with all community providers, including attempts to reach providers and/or schedule outpatient appointments.
- Secures collateral information requested by psychiatrist after admission process is completed.
- Utilizes case management skills, such as, advocacy, transition and discharge planning to link clients to resources and services. Coordinates referrals to community and internal resources, such as community psychiatrists, therapists or outpatient programs.
- Notifies recovery team of any changes in clients mental status and/or behavior, maintaining open communication among client, recovery team, psychiatrist and family, as necessary.
- Links or provide services for clients in the community such as transportation needs, appointments with potential post-discharge providers, scheduling doctors appointments, etc.
- When appropriate, assists, teaches, and models daily living skills, healthy boundaries, and communication skills.
- Attend weekly supervision with Chief Medical Officer to review client cases, clinical concerns, and training. Attend weekly scheduled supervision with Director of Clinical Services for administrative supervision, direction, and case consultation.
- Attend community meetings, client engagement meetings, and mandatory in-services hosted at Skyland Trail, as well as other clinical trainings and learning opportunities deemed important by clinical supervisors.
- Comply with facility policies and accreditation standards (i.e., Joint Commission, HRFD, ORS, CCI
- Participate in continuing education programming in accordance with state licensing body and goals of specific recovery communities assigned.

Qualifications
Qualifications:
- Masters degree in related field (i.e.; Psychology, Social Work, Counseling, etc.) and/or two (2) + years of clinical experience in the mental health field.
- Prior case management experience preferred
- Prior group facilitation skills required
