About the role

We are seeking an experienced Grant and Proposal Writer with a strong track record developing successful grant applications, competitive proposals, and strategic responses to funding opportunities for a part-time role (15-20 hours per week). This role is responsible for researching, writing, and managing the full lifecycle of grant applications, RFP responses, and strategic proposals—from opportunity identification through submission and post-award reporting with a primary focus on Medicare, Medicaid, Community Health Worker (CHW) programs, rural health transformation initiatives, and healthcare partnerships.

These activities will help Pear Suite and its nonprofit, community-based organization (CBO), healthcare provider, health plan, and government partners secure funding, launch new programs, and expand access to community-based care. The role will support proposals that strengthen partner organizations while advancing shared healthcare goals. The ideal candidate has hands-on experience navigating federal funding portals (e.g., Grants.gov, SAM.gov), interpreting Notices of Funding Opportunity (NOFOs) from agencies such as CMS and HRSA, and crafting compelling, compliant narratives that align organizational programs with federal health policy priorities, including value-based care, health equity, workforce development, and rural health access.

What you'll do
Research & Strategy
  • Monitor and identify relevant federal funding opportunities across agencies such as the Centers for Medicare & Medicaid Services (CMS), Health Resources and Services Administration (HRSA), Center for Medicare and Medicaid Innovation (CMMI), Office of Rural Health, and related HHS sub-agencies
  • Monitor strategic procurement, payer, provider, and partnership opportunities requiring formal proposal responses
  • Track opportunities tied to CHW workforce development, Medicaid/Medicare demonstration and innovation models, rural health transformation programs (e.g., Rural Health Transformation initiatives, Rural Communities Opioid Response Program, Rural Health Care Services Outreach), and value-based/alternative payment model funding
  • Analyze NOFOs, RFPs, RFIs, and other solicitation documents to assess eligibility, competitiveness, and strategic alignment, making recommendations on whether Pear Suite should pursue each opportunity
  • Stay current on CMS/CMMI policy shifts, Medicaid state plan amendments, and rural health funding landscape changes that affect proposal strategy
  • Maintain a rolling calendar of state, federal, and foundation application deadlines and internal milestones
  • Write clear, persuasive, and technically compliant grant narratives, including statements of need, program design, logic models, evaluation plans, and sustainability sections
  • Translate program and technical information from subject-matter experts into compelling proposal language
  • Develop grant applications, RFP responses, RFIs, and strategic proposals that support Pear Suite, its nonprofit and CBO partners, healthcare providers, health plans, government agencies, employers, and other healthcare organizations
  • Develop and coordinate required attachments: budgets, budget narratives, timelines, organizational capacity statements, letters of support, and required federal forms (e.g., SF-424 family of forms)
  • Ensure full compliance with agency-specific formatting, page limits, and submission requirements
  • Maintain reusable proposal content, case studies, organizational narratives, and supporting materials to improve future proposal quality and speed
  • Review solicitation requirements to identify compliance risks, submission requirements, and evaluation criteria
  • Coordinate proposal timelines, review cycles, and submission checklists across internal stakeholders
Cross-Functional Coordination
  • Partner with internal teams and external nonprofit, CBO, clinical and healthcare partners to gather accurate data, outcomes, budgets, and supporting materials
  • Coordinate with finance/accounting on budget development and indirect cost rate application
  • Manage internal review cycles and incorporate feedback from multiple stakeholders under tight deadlines
Compliance & Submission
  • Prepare and submit grant applications, proposal responses, and required documentation through Grants.gov, agency-specific portals, procurement systems, and other submission platforms
  • Track registration renewals (SAM.gov, UEI, etc.) and ensure organizational eligibility documentation stays current
  • Maintain thorough records of submissions, correspondence, and compliance documentation
Post-Award Support
  • Assist with post-award reporting requirements, progress reports, and no-cost extension requests as needed
  • Support audit readiness by maintaining organized proposal and compliance files Describe the specific responsibilities and job functions of the role
Qualifications
Required Qualifications
  • 5+ years developing successful grant applications, RFP responses, or strategic proposals, with demonstrated success supporting federal healthcare funding and/or healthcare procurement opportunities
  • Working knowledge of the Medicaid and Medicare program landscape, including value-based payment models, health equity initiatives, and rural health delivery challenges
  • Direct experience writing successful applications to CMS, CMMI, HRSA, state Medicaid agencies, or comparable healthcare procurement and proposal opportunities
  • Proven experience navigating Grants.gov, SAM.gov, and federal application systems
  • Strong understanding of federal compliance requirements (e.g., 2 CFR 200 / Uniform Guidance) and HHS/CMS-specific grant terms and conditions
  • Excellent writing, editing, and storytelling skills with the ability to translate complex clinical, programmatic, and policy information into clear, persuasive narrative
  • Strong project management skills and ability to manage multiple deadlines simultaneously
  • Experience developing budgets and budget narratives in collaboration with finance teams
  • Experience supporting healthcare RFPs, procurement responses, managed care proposals, or strategic partnership submissions
Preferred Qualifications
  • Familiarity with Community Health Worker workforce models, scope of practice, certification/training frameworks, and CHW reimbursement pathways
  • Experience supporting Rural Health Transformation Program applications or similar state/federal rural health initiatives
  • Familiarity with CHW integration models within Medicaid managed care, health systems, ACOs, or FQHCs
  • Understanding of Medicaid Section 1115 waivers, Medicare Innovation Center demonstration models, or rural health redesign frameworks
  • Familiarity with logic models, theory of change frameworks, and health outcomes/quality measure evaluation design (e.g., HEDIS, UDS measures)
  • Prior experience in a healthcare, public health, and/or rural health system
  • Track record of securing awards of $500K+
Compensation
  • The hourly rate for this role is $45-$55, dependent upon level of experience and geographic location.
