Job summary

Regency Integrated Health Services is seeking a GN/GVN Graduate Nurse to join the Bastrop Lost Pines Nursing and Rehabilitation Center on a PRN basis. The role supports RN supervision, assists in quality care planning, and helps with assessments and care documentation.

The ideal candidate will have completed an RN/LVN program and be pursuing Texas licensure, with preferred experience in skilled nursing; strong communication and teamwork are essential.

Qualifications

• Has completed an RN/LVN program at an accredited vocational school or college.
• Must be working toward testing for a Registered Nurse/Vocational Nurse license in Texas.
• Experience in a skilled nursing facility preferred.
• Excellent observation, documentation, and communication skills.

Responsibilities

• During rounds observes, assesses, documents, and reports resident condition/changes.
• Assures implementation of care plans as directed.
• Administers medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Identify significant changes in resident condition and take action per policy.
• Oversees and monitors aides.
• Discharge residents per facility policy.

Overview

Position Type: PRN | Location: Bastrop Lost Pines Nursing and Rehabilitation Center, Bastrop, TX 78602. The GN Graduate Nurse/GVN Graduate Vocational Nurse, under the supervision of an RN, will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.

Responsibilities

• During rounds observes, assesses, documents, and reports resident condition/changes
• Assures implementation of care plans as directed
• Administers medications as prescribed
• Conduct rounds with physician and record visits in clinical records
• Perform narcotic reconciliation with incoming and outgoing shifts
• Identify significant changes in the condition of residents and take necessary action per facility policies and procedures
• Oversees and monitors aides
• Initiate and complete Incident and Accident Reports per facility policy
• Discharge residents per facility policy
• Must complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition
• Communicate with resident’s point of contact when they ask for a status update or if there is a change in condition
• Other special projects or functions can be assigned at times by the Director of Nurses and/or the Administrator

Qualifications

Licensing

• Must be working toward testing for a Registered Nurse/Vocational Nurse license in the state of Texas

Experience

• Experience working in a skilled nursing facility is preferred
