This position involves contacting identified members to inform and educate them about health care programs tailored to their personal health plan needs. It includes comprehensive respiratory care coordination for customers diagnosed with chronic obstructive pulmonary disease (COPD) and/or asthma. The role also entails engaging members in discussions about adherence to personal health plans, responding to their inquiries, and supporting clinicians in the Medical Management department with provider and member activities.
