Qualifications

• Active nursing license (RN or LPN) and up-to-date physical.
• BLS CPR certification.
• QuantiFERON or PPD test.
• Ability to work in a home care setting.

Responsibilities

• Provide direct patient care in home settings.
• Report changes in patient condition to supervising RN.
• Maintain clear communications with staff, patients and families.
• Ensure knowledge of medications, dosages and reactions.
• Be punctual and dependable for all shifts.
• Participate in ongoing education and in-services.

General Responsibilities

• Punctuality and Dependability: Show up on time and be reliable for all assigned/confirmed shifts.
• Patient Accountability: Take full responsibility for each assigned patient under the supervision of the supervising RN.
• Medication Knowledge: Stay informed about all medications, reactions, and dosage requirements for your patient.
• Communication: Maintain excellent written and verbal communication with staff, patients, and families.
• Continuous Learning: Engage in educational tools and in-services to expand your knowledge of diseases, age-specific care, and best practices.
• Direct Patient Care: Provide thorough, compassionate care and report any changes in the patient's condition to the supervising care team to ensure comfort and safety.
• Collaborative Care: Assist physicians and healthcare professionals in caring for the patient, providing detailed reports at the end of each shift to the oncoming nurse.
• Other Duties as Assigned: Support the care team with additional responsibilities as needed
