Job summary

Responsibilities

• Delivers and manages care for outpatient Veterans, including Fibroscans.
• Coordinates comprehensive pre-procedure assessments, educates patients and families, and ensures post-procedural care compliance.
• Collaborates with interdisciplinary teams to align patient care with the Veteran's goals and overall treatment plan.
• Develops, implements, evaluates, and revises clinical policies, procedures, and guidelines based on evidence-based practice.
• Provides peer feedback and fosters a professional environment that supports continuous learning and development.
• Patient intake, chart preparation, care management, secure messaging, phone triage, procedural care and vitals.
• Assesses, plans, implements, and evaluates care based on age-specific components.
• Triages consults and patients as needed. Provides coverage to all clinics within the Specialty Department.
• Administers medications and procedures per established policies and guidelines. Medication administration using Bar Code Medication Administration (BCMA).
• Leads within a High-Reliability Organization framework with a culture of safety to support health care solutions based on prevention, no punishment, with collective mindfulness.
