We are currently seeking a compassionate and experienced registered nurse (RN) to join our clinical team. This fully remote position is ideal for nurses who are passionate about patient care and eager to make an impact without the burden of excessive documentation. Using our proprietary software platform, you’ll engage with patients and provide ongoing clinical support in a fully remote setting.

Responsibilities:

• Review patient vitals and care plans within our proprietary technology platform
• Develop and manage personalized care plans addressing clinical, social, and behavioral needs
• Respond to clinical alerts to support improved health outcomes
  
• Provide patient education and guidance via video, phone, and asynchronous communication
  
• Coordinate care with physicians and act as a liaison between patients and providers
  
• Support patients in building healthier habits over time

Requirements:

• Active registered nurse (RN) license in the state of Illinois
• Compact state license
  
• Minimum of 3 years of experience in GI (gastrointestinal) care
  
• Comfortable using technology in a remote work environment
  
• Excellent communication and customer service skills

Preferred Qualifications:

• Experience in case management
• Knowledge of motivational interviewing techniques
• Bilingual or multilingual proficiency
