Job summary

Qualifications

• Graduate degree required from an accredited PA or NP program.
• 1 year of recent and related experience as an Advanced Practice Provider preferred.
• Colorado PA license or APRN license and prescriptive authority. DEA Registration. PA or NP certification by a national certifying body. Current CPR/BLS certification is required. ACLS or PALS certification if applicable for specific practice area.

Responsibilities

• Provides regular health assessments, diagnosis, and management of acute and chronic conditions and formulate plans that include treatment, psychosocial support, health education, and counseling.
• Perform and document history and physical exams, gather data from medical records, review outside records, communicate with referring providers, and develop and implement the plan of care.
• Monitor and document progress, revise plan of care as needed, interpret and monitor test results, and intervene when specialty referral or escalation is needed.
• Act as a consultant and present current educational programs to community groups, organizations, or individuals.
• Contribute to education and research through participation in research or quality improvement projects and precepting.

Education
