Qualifications

• Board Certified/Board Eligible in Geriatrics (preferred).
• FM or IM with experience in complex care management.
• Strong multidisciplinary collaboration across care teams.

Responsibilities

• Provide full-time post-acute and long-term care in community sites with ~11–13 patients per day.
• Manage admissions, transitions of care, discharge planning, and chronic primary care.
• Collaborate with a growing team of physicians, APPs, nurses, and therapists.
• Engage in teaching and potential research opportunities.
