Qualifications

• Must be BC/BE in Geriatrics, Internal Medicine, Palliative Care/Hospice or Family Medicine.
• MD/DO from accredited US medical school.
• Residency completed at an accredited institution or US residency/5-year pathway.
• Active NY License.
• Current New York State Medical Practitioner License.
• Current DEA and DPS registration for prescribing.

Responsibilities

• Provide primary medical care for Nursing Home patients.
• Order labs and radiology tests and refer to specialists when needed.
• Analyze medical records and examinations to diagnose conditions and propose treatments.
• Prescribe medications and treatment regimens.
• Advise patients on lifestyle and diet changes to improve health.
• Maintain patient confidentiality and comply with privacy laws.
• Document medical records timely and accurately.
