Qualifications

• 1 year of post-graduate clinical experience with geriatric population preferred.
• Experience in palliative care or hospice valuable.

Responsibilities

• Provide primary care in diverse settings including outpatient clinics and community-based sites.
• Collaborate with physicians, nurse practitioners, social workers, and other team members.
• Deliver patient-centered, whole-person care for older adults.
• Support care coordination across medical and behavioral health services.
