Job summary

Providence PACE (Program of All-Inclusive Care for the Elderly) is seeking a full-time physician to join our Portland area team. This primary care role emphasizes team-based care with geriatrics and palliative care, delivering comprehensive, whole-person services in varied settings.

You’ll work with an interdisciplinary team and provide care in outpatient clinics, private residences, assisted living facilities, and SNFs, helping older adults stay active and independent.

Qualifications

• 1 year of post-graduate clinical experience working with a geriatric population preferred.
• Training and/or experience in palliative care and/or hospice valuable.

Responsibilities

• Provide care in outpatient clinics and in a variety of community-based settings including private residences, assisted living facilities, adult care homes, and skilled nursing facilities.
• Collaborate closely with physicians, nurse practitioners, nurse care managers, social workers, pharmacists, psychiatry/mental health clinicians, therapists, and other interdisciplinary team members.
