Job summary

Rochester Regional Health is seeking a Nurse Practitioner for ElderONE, the organization's PACE program, to deliver comprehensive, home- and community-based care to older adults with complex needs. You’ll collaborate with an interdisciplinary team to create individualized plans that support independence and quality of life.

The role is full-time in the Geriatrics department with a Monday–Friday, 8:00 AM–4:30 PM schedule.

Responsibilities

• Clinical Care: Evaluate and manage acute, sub-acute, and chronic needs of older adults.
• Care Management: Complete comprehensive reviews and coordinate follow-up care.
• Interdisciplinary Team Administrative Duties: Attend daily meetings and care planning conferences; review results.
• PACE & Community-Based Care: Support 800+ ElderONE participants across multiple sites and homes.
• Program Growth & Collaboration: Contribute to growth and quality improvement initiatives.
• Infection Control: Apply safety guidelines to prevent infections.
