Job summary

Vitality NP Services is seeking a Nurse Practitioner to join a Skilled Nursing Facility in Haskell, NJ. This in-person role offers part- or full-time hours with weekend and weekday flexibility, no evenings, weekends, holidays, or on-call requirements.

Base salary ranges from $120,000 to $170,000 per year plus a productivity bonus. The position emphasizes geriatrics and primary care, with a strong focus on admission assessments and preventing hospital transfers.

Qualifications

• Previous RN experience required; New NP graduates welcome to apply
• Strong critical thinking and assessment skills
• Ability to thrive in a fast-paced environment
• Excellent written and verbal communication skills
• Comfortable with electronic documentation and computer systems

Responsibilities

• Perform comprehensive assessments for new admissions and acute issues in subacute and long-term care patients
• Prevent avoidable hospital transfers through early evaluation and timely intervention
• Collaborate and communicate effectively with primary care providers and specialists
• Participate as an active member of the interdisciplinary care team
