Job summary

Petaluma Health Center seeks an Internal Medicine/Family Medicine physician to deliver high-quality primary care to adult and elderly patients in a compassionate setting. The physician will participate in strategic initiatives and may be assigned to after-hours, weekend clinics, and school-based or facility sites within PHC’s scope.

Responsibilities include comprehensive patient care, coordination with the medical staff, supervision of mid-level providers, and ensuring timely access to services

Qualifications

• Graduate from an accredited medical school.
• Completion of an accredited residency program.
• Board Certified in Internal Medicine or Family Medicine with current CA License and DEA.
• Board Certified in Geriatrics or evidence of enhanced education and/or 5+ years of geriatric experience.

Responsibilities

• Diagnose and treat patients within licensure, and provide follow-up care.
• Provide geriatrics consultation to PHC providers across sites.
• Coordinate transitions of care and post-acute care planning.
• Supervise PA/NP staff and collaborate with medical team.
• Maintain credentials and participate in clinic quality improvement.
