Job summary

Abbotsford Health Care Center in Abbotsford, WI, is seeking a dedicated Physical Therapist to evaluate, treat, and document care for residents with limited daily living abilities. You will work within an interdisciplinary team to restore function and independence through restorative programs and therapeutic interventions.

You will assess needs, develop treatment plans, and educate residents and families while maintaining compliance with facility policies and regulatory requirements.

Qualifications

• Excellent verbal and written communication with residents, families, and healthcare professionals.
• Ability to read, analyze, and interpret common medical information.
• Proficient with Word and Excel.

Responsibilities

• Evaluate and document evaluations of residents according to facility policies.
• Provide appropriate treatment to help residents regain highest possible function.
• Recommend and provide adaptive equipment to prevent decline in function.
• Educate residents and caregivers to support ongoing independence.
• Maintain timely, accurate resident reports for regulatory agencies and payers.
• Observe infection control procedures and ensure resident safety.
• Complete MDS assessments and related documentation.
• Participate in care conferences and rehabilitation planning.
