Palo Alto Foundation Medical Group in the San Francisco Bay Area seeks BE/BC physicians in internal medicine, family medicine, or geriatrics for a growing Post-Acute program in Fremont, CA. The role offers schedule flexibility and a physician-led environment.

You will provide post-acute and long-term care in skilled nursing facilities, supported by a nurse triage team and opportunities for leadership development within a growing geriatrics group.

Qualifications

• Current California Medical License.
• DEA certification is required.
• BE/BC in Internal Medicine or Family Medicine with geriatrics focus.
• Fellowship in Geriatric Medicine completed.

Responsibilities

• Post-acute and long-term patient care in skilled nursing facilities.
• Collaborate with a nurse triage team to coordinate care.
