Job summary

St. John's Community Health in Los Angeles is seeking a compassionate Geriatrician to deliver comprehensive primary and preventive care to older adults. The role focuses on cognitive assessments, chronic disease management, and coordination of multidisciplinary teams.

You will conduct geriatric assessments, address dementia and frailty, support advance care planning, and supervise Advanced Practice Providers while engaging with diverse aging populations in a faith‑based community context.

Qualifications

• MD or DO degree is required and CA license is expected.
• Residency in Internal Medicine or Family Medicine completed.
• Board certification or eligible in IM or FM.
• Geriatric Medicine fellowship is preferred.

Responsibilities

• Provide comprehensive primary and preventive care for older adults.
• Manage acute and chronic medical conditions.
• Conduct geriatric, cognitive, and functional assessments.
• Evaluate and manage dementia, frailty, and age-related conditions.
• Perform medication reconciliation and address polypharmacy.
• Collaborate with specialists, behavioral health, social workers, and caregivers.
• Support advance care planning and goals-of-care discussions.
• Document care in the EHR and participate in quality improvement.
• Supervise Advanced Practice Providers as assigned.

About The Role

SJCH is seeking a compassionate Geriatrician to provide comprehensive, patient-centered care to older adults. This role focuses on preventive care, chronic disease management, cognitive assessments, medication management, fall prevention, advance care planning, and coordination of care across multidisciplinary teams.

Key Responsibilities

• Provide comprehensive primary and preventive care for older adults
• Manage acute and chronic medical conditions
• Conduct geriatric, cognitive, and functional assessments
• Evaluate and manage dementia, depression, frailty, and age-related conditions
• Perform medication reconciliation and address polypharmacy concerns
• Collaborate with specialists, behavioral health providers, social workers, pharmacists, and caregivers
• Support advance care planning and goals‑of‑care discussions
• Document patient care accurately in the EHR
• Participate in quality improvement and community outreach initiatives
• Supervise Advanced Practice Providers as assigned

Additional Requirements

• Experience providing care within faith-based or congregational settings, including partnerships with faith communities and religious institutions.
• Ability to support health education, screenings, wellness monitoring, care coordination, and community-based outreach initiatives.
• Willingness and ability to conduct home visits and provide care outside of a traditional clinical setting.
• Demonstrated cultural competency in caring for diverse aging populations, with an understanding of health disparities, social determinants of health, and the needs of underserved elderly communities.

Qualifications

• MD or DO degree
• Completion of an accredited Internal Medicine or Family Medicine Residency
• Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Fellowship in Geriatric Medicine preferred
• Active California Medical License, DEA, and BLS Certification
• Experience caring for geriatric populations preferred
