Job summary

Eventus WholeHealth seeks licensed clinicians to provide recovery-oriented rehabilitation services in long-term care facilities. You will work within a multidisciplinary team to address anxiety, depression and complex mental health needs, delivering care in group or individual settings.

We emphasize HIPAA, documentation, and collaboration with facility staff while maintaining high ethical standards and patient confidentiality in a compassionate care environment.

Qualifications

• Master’s degree in social work and a current license to practice in the state.
• Licensed Clinical Social Worker (LCSW) or equivalent license required in the state of practice.

Responsibilities

• Assess residents to determine needs and develop treatment plans.
• Consult with primary care physicians, family, facility staff as needed.
• Document treatment plans and sessions per policy; provide timely documentation.
• Coordinate with facility staff to review care needs on arrival and departure.
• Maintain communications with facility staff and provide guidance on treatment plans.
