Job description

Position Details:

Type: Full-time, 35 hours per week

Work Arrangement: Hybrid up to 3 days per week once training is complete

Job Responsibilities:

1. Function as a case manager within the Primary Care Team (PCT) for members participating in a Senior Care Option (SCO) plan.
2. Make recommendations, coordinate, implement, and monitor contracted and community services to improve or maintain members' overall health and functional status.
3. Conduct periodic reassessments to determine the appropriateness of the service plan.
4. Ensure members have access to public benefits to support wellness and independence, with the goal of preventing premature institutionalization.

Additional Details:

• Travel reimbursement is $0.62/mile.
• Minimum Qualifications: BA or BS in Social Work, Human Services, or related field; experience in case management with geriatric population; LSW a plus.
• Must have a valid driver's license and/or reliable transportation.
• Fluency in Spanish and English required.
• Ability to climb multiple flights of stairs.
