Greenspring Village by Erickson Senior Living is seeking a full-time, resident-centered Social Worker to support our 62-resident Skilled Nursing neighborhood. You will provide social work services across skilled nursing, long-term rehab, assisted living, and memory care, onsite with one weekend day required.

Responsibilities include psychosocial assessments, admission-to-discharge coordination, care planning collaboration, and bereavement support, while complying with state and federal

Qualifications

• Social Work License for Virginia preferred.
• Social Work Degree is required; MSW preferred.
• Minimum 3 years’ experience with older adults; geriatric/dementia knowledge required.
• Minimum 2 years experience in Skilled Nursing / Long Term Care.
• Experience with EHR / EMR preferred; proficiency with Word and Outlook.

Responsibilities

• Completes psychosocial assessments, MDS, and required documentation per policy and regulations.
• Coordinate Social Work from admission through discharge, including care plan and family communication.
• Provide bereavement support and monitor psychosocial needs; ensure documentation in medical records.
