Job summary

The Admiral at the Lake in Chicago seeks a Social Worker – IL to support Independent Living residents. You will conduct psychosocial and cognitive assessments and coordinate the pre-admission process, helping residents navigate health, finances, and community resources.

You will coordinate care, support discharge planning, arrange home care when needed, and maintain EMR documentation while collaborating with residents, families, and providers to promote independence and well‑being.

Qualifications

• MSW degree required.
• Two years of experience with older adults required.
• Healthcare, senior living, hospital or related setting experience preferred.
• Experience with dementia or cognitive impairment required.
• Licensed Clinical Social Worker (LCSW) preferred.

Responsibilities

• Coordinate pre-admission process and psychosocial assessments.
• Identify services and resources for residents and families.
• Help with discharge planning and coordination of community resources.
• Maintain documentation in EMR and support wellness initiatives.
• Liaise between residents, families, departments and outside providers.
