Job summary

Windsor Gardens Care Center seeks a Social Worker to plan and administer comprehensive social services for residents and families. You will coordinate discharge planning, collaborate with the treatment team, and maintain records to support holistic care.

The role involves liaising with residents, families, and outside agencies, ensuring residents’ rights are upheld and care plans are followed. Strong interpersonal and regulatory knowledge are essential.

Qualifications

• Bachelor’s degree in Social Work or a bachelor’s degree in a human services field including, but not limited to, Sociology, Special Education, Rehabilitation Counseling and Psychology.
• Any certifications required by New Jersey State regulations.
• Knowledge of family assessment and interventions is preferred.

Responsibilities

• Plan and administer social services programs for residents and families in the facility.
• Coordinate discharge planning and community resources.
• Maintain records of residents’ social work needs and care.
• Liaise with residents, families, outside agencies, and staff to ensure rights are maintained.
• Ensure safety and adherence to policies while supporting residents’ wellbeing.
• Conduct family meetings to discuss discharge planning and care plans.
