Guardian Angels Senior Services in Elk River, MN is seeking a Social Worker to assist with the social services program, focusing on improving the well-being of residents within the Transitional Care Unit.

This full-time role requires a dedicated individual with a degree in social work and Minnesota licensure, who possesses excellent communication skills and experience in geriatric care.

Qualifications

• Must be a graduate of an accredited college, and have current Minnesota licensure as a Social Worker.
• Experience in geriatric social work and willingness to work with geriatric residents is required.
• Must have excellent verbal and written (read and write) communication skills.
• Must maintain good attendance and punctuality.

Responsibilities

• Assist in the admission process, including completion of admission paperwork.
• Actively assist with discharge planning starting on admission with transitional care residents.
• Participate in the development of a written, interdisciplinary plan of care for each resident.
• Provide support to team for identification of mood and behavioral expressions.
• Coordinate the resident discharge planning process and make referrals for appropriate home care services.
