Qualifications

• Bachelor's degree in Social Work or related field.
• Minimum 1 year experience in social work or geriatric population.

• Compassion, empathy, and understanding of the needs of seniors and their families.

Responsibilities

• Provide social services to residents, including counseling, therapy, and support groups.
• Conduct assessments and develop individualized plans to meet the needs of each resident.
• Facilitate communication and collaboration between residents, families, and healthcare professionals.
• Identify and address psychosocial and emotional needs and develop strategies to improve residents' well-being.
• Assist residents in adjusting to new living environments and transitions.
• Participate in interdisciplinary team meetings to develop care plans.
• Maintain accurate records of resident services and progress.
