Job summary

Element Care in Boston is seeking an experienced Social Worker to participate in planning, implementation and evaluation of care plans within the PACE model. The social worker helps older adults live safely and comfortably at home and in their communities.

You will join the IDT, conduct assessments, provide counseling, support admissions and discharges, and coordinate with guardians, Medicaid specialists and family caregivers. This is a full-time role requiring MA MSW licensure and CPR.

Qualifications

• Licensed MSW in MA (LISW or LCSW) required.
• Minimum 1 year of geriatric social work experience.
• Current CPR certification or ability to obtain.
• Strong communication and multitasking in a fast-paced environment.

Responsibilities

• Participates in IDT assessments and care planning.
• Attends IDT meetings and supports care plans.
• Completes initial, semi-annual, annual assessments and service requests.
• Provides psychosocial counseling with behavioral health providers.
• Manages hospital/NH admissions and PASRR documentation.
• Maintains social work documentation in medical records.
