Job summary

Arborviewsl in Colorado is seeking a Social Services Specialist to help residents and families adjust to placement and plan of care. You will support in evaluating social, emotional, and financial needs and work within federal and state regulations while aligning with company policies.

Responsibilities include interviewing families, coordinating services, ensuring resident rights are respected, and assisting with discharge planning and community resources.

Qualifications

• Experience with geriatric population or long-term care setting preferred.
• Strong communication and interpersonal skills.
• Empathy and compassionate patient-centered approach.
• Professional conduct and ethics in handling resident needs.
• Ability to explain plans and resources clearly to families.

Responsibilities

• Interview residents and families to gather psychosocial information for care plans.
• Assist with transition and adjustment to placement in the facility.
• Respect resident rights and document preferences including DNR and treatment wishes.
• Inform residents and families about personal and property rights.
• Support in identifying social, financial, legal, and community resources.
• Coordinate appointments and transportation for residents.
• Assist with discharge planning and follow-up arrangements.
