Job summary

Telos Health Systems, Inc. is seeking an experienced Advanced Practice Provider (NP or PA) to serve as the in-house SNF-ist in Jamestown, KY. You will deliver general medicine care to geriatric residents across SNFs and ALFs, with autonomy and collaborative support.

The role offers a flexible schedule, W-2 employment, and a hybrid model combining in-person care with virtual charting, designed to improve patient outcomes and reduce unnecessary hospitalizations.

Qualifications

• Active, unencumbered KY APRN/PA license.
• Master’s degree from an accredited NP/PA program.
• DEA registration and licensure requirements must be current.

Responsibilities

• Serve as the facility’s primary internal medicine provider for SNF/ALF residents.
• Manage care for short-term rehab and long-term residents.
• Perform comprehensive health assessments and physical exams.
• Manage acute and chronic conditions per established protocols.
• Prescribe medications and treatments as appropriate.
• Conduct monthly stability visits and coordinate with interdisciplinary teams.
