Job summary

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide patient-centered treatment and care.
• Demonstrates exceptional patient service and clinical excellence.
• Adheres to scheduled hours and professional conduct.
• Compliance with confidentiality, HIPAA and Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred.

Responsibilities

• Assessment of referred patients requiring rehabilitation and direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Document patient conditions and educate patients and families.
• Collaborate with an interdisciplinary team on facility-wide programming and policies.
• Maintain HIPAA-compliant records and ensure professional conduct.
