Job summary

Meadow Park Rehabilitation and Health Care Center in Queens, NY, is seeking a full-time Speech-Language Pathologist (SLP) to join our rehab team. You will evaluate residents, develop individualized treatment plans, and provide interventions to improve communication, cognition, and swallowing.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current and valid New York State license to practice Speech-Language Pathology.
• CCC-SLP certification is preferred.
• Experience in a nursing home, skilled nursing facility, rehabilitation center, or similar healthcare setting preferred.
• Strong clinical judgment, communication, organizational, and documentation skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Compassionate, dependable, and committed to high-quality resident care.

Responsibilities

• Perform comprehensive speech, language, cognitive-communication, and swallowing evaluations.
• Develop and implement individualized treatment plans based on each resident’s clinical needs and goals.
• Provide therapeutic interventions to improve communication, cognition, and swallowing function.
• Assess swallowing difficulties and provide appropriate treatment recommendations and swallowing precautions within the scope of practice.
• Monitor resident progress and adjust treatment plans as needed.
• Maintain accurate and timely clinical documentation in accordance with facility policies and applicable regulations.
• Collaborate with physicians, nursing staff, dietary services, rehabilitation professionals, and other members of the interdisciplinary team.
• Participate in resident care planning and communicate clinical findings and recommendations.
• Educate residents, families, and caregivers on treatment strategies, communication techniques, and swallowing safety.
• Ensure compliance with applicable New York State regulations, professional standards, and facility policies.
