Caring Angles Home Care is seeking a dynamic Geriatric RN Case Manager with Oasis experience for our Brooklyn office. As Case Manager you will coordinate patient care, supervise clinical staff, and ensure services are delivered appropriately.

You will review referrals, assess needs, and guide clinicians to improve outcomes. The role emphasizes coordination, compliance with standards, and active participation in community activities to support the organization’s health care delivery mission.

Qualifications

• Current New York State Registered Nurse license.
• Experience in geriatric home care and Oasis.
• Strong case management and coordination skills.

Responsibilities

• Coordinate and manage patient care.
• Supervise clinical personnel and ensure appropriate delivery of services.
• Receive case referrals and assess home care needs based on patient data.
• Review and evaluate cases, guide clinicians, and be available during operating hours.
• Review patient diagnoses, medications, procedures, and clinical course.
• Assist clinicians in setting immediate and long-term goals and plan of care.
• Attend case conferences to facilitate care coordination.
• Conduct quarterly record reviews and report findings to DPS.
• Comply with professional standards and principles.
• Engage in community activities to promote the organization’s role in health care delivery.
• Promote customer service orientation among staff.
• Execute other duties as delegated by the DPS.
