New York Community Care in Brooklyn is seeking a dynamic Geriatric RN Case Manager with Oasis experience to join our team in our Brooklyn office. You will play an integral role in coordinating care and supervising clinical staff to deliver quality home health services.

Qualifications

RN license required in New York State.
Oasis experience preferred.
Experience supervising clinical staff.
Strong coordination of care and communication skills.

Responsibilities

Ensure care/services are delivered appropriately and supervise clinical personnel.
Receive case referrals and assess home care needs from patient information.
Review services, guide clinicians to improve care quality and be available during operating hours.
Review patient diagnoses, medications, procedures, and clinical course.
Assist clinicians in setting priorities and developing plans of care.
Attend case conferences to coordinate care.
Conduct quarterly chart reviews and communicate findings to DPS.
Ensure compliance with professional standards.
Participate in public relations and community activities promoting the organization.
Promote customer service orientation among staff.
Perform other duties as delegated by the DPS.
