Tender Touch Rehab Services seeks a compassionate Physical Therapist Assistant to provide high-quality rehabilitation services for residents in skilled nursing and long-term care facilities.

Under the direction of a licensed Physical Therapist, you will implement individualized treatment plans, lead therapeutic exercises, gait and balance training, and functional mobility activities to help seniors maximize independence and quality of life.

Qualifications

• Associate degree in Physical Therapist Assistant.
• Compassionate patient-focused approach.
• Ability to work under supervision of a licensed PT.

Responsabilités

• Provide rehabilitation services in SNF/long-term care facilities.
• Implement individualized treatment plans and therapeutic exercises.
• Gait and balance training and functional mobility activities.
• Document progress in the EMR and educate residents and families.
• Collaborate with an interdisciplinary team to support patient goals.
