Job summary

Hale Ho Aloha, Inc. seeks a qualified Physical Therapist Assistant to plan and deliver a resident-focused functional maintenance exercise program. You will work under the supervision of a Physical Therapist to provide direct patient care in a skilled nursing setting.

The role requires an AS in PTA, Hawaii licensure, and current CPR/BLS certification. Minimum of one year experience is required; long-term care experience is preferred.

Qualifications

• Graduate of an accredited PTA program with AS degree.
• Current Hawaii PTA licensure.
• CPR/BLS certification with retraining every two years.

Responsibilities

• Plan, organize, and assist with a resident's functional maintenance exercise program under supervision.
• Deliver direct physical therapy services to residents as required by the supervising Physical Therapist.
• Document daily participation and progress per company guidelines.
• Provide clinical direction to restorative nurse aides related to the exercise program.
• Instruct residents in ADLs, body mechanics, safety, and use of adaptive devices as needed.
• Participate in interdisciplinary resident care plan meetings and maintain required documentation.
• Maintain confidentiality and resident rights; ensure a safe environment and equipment.
