Crystal Heights in Des Moines is seeking a full-time Physical Therapist for our in-house rehabilitation program. You will conduct screenings, develop care plans, and deliver skilled therapies to geriatric patients in a post-acute setting.

As part of a collaborative team, you will provide counseling to patients and families, document outcomes, and participate in discharge planning, with a focus on CAPLICO values in daily practice.

Qualifications

• Physical Therapist license is required.
• Open to Physical Therapists at all experience levels.
• New Grads are welcome!

Responsabilités

• Screen and evaluate patients with physical functioning disorders and wound care; create care plans.
• Deliver rehabilitation and skilled treatment interventions.
• Perform manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
• Provide counseling to patients, families, caregivers and other providers.
• Generate comprehensive discharge summaries in compliance with regulations.
