Job summary

Anaheim Point Healthcare & Wellness Center seeks a licensed Physical Therapist to provide direct patient care and contribute to therapy outcomes.

You will evaluate patients, develop treatment plans, supervise assistants, and document care in medical records, while collaborating with families and physicians. The role emphasizes safe practices, professional development, and quality improvement.

Qualifications

• Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
• Experience in geriatric rehabilitation preferred.
• Experience in adult neurological, cardiac, and/or general rehab preferred.
• Current Physical Therapist Registration/State Licensure as required.

Responsibilities

• Provide direct patient care and treatments in line with facility procedures.
• Supervise PT assistants and aides during patient care.
• Document evaluations, progress notes, and discharge summaries.
• Communicate with families, physicians, and the interdisciplinary team.
• Educate families and staff on prescribed rehab plans.
