Job summary

Qualifications

• Graduate of an accredited Physical Therapy program.

Responsibilities

• Initiate and implement personalized physical therapy programs based on physician orders.
• Maintain detailed evaluations, progress notes, and discharge summaries.
• Collaborate with nursing and rehab staff to enhance residents' functional outcomes.
• Contribute to interdisciplinary care planning meetings.
• Maintain treatment documentation, track patient statistics, and request necessary supplies.
• Participate in or lead staff training and education.
• Provide leadership support in the absence of the Director of Physical Therapy.
