Job summary

BoldAge PACE in California is seeking a dedicated Physical Therapist to implement personalized therapy services for our participants. Responsibilities include conducting assessments, planning treatment programs, and educating families and staff.

The ideal candidate will have an advanced degree in Physical Therapy and a minimum of two years of experience working in an interdisciplinary team, preferably in a community-based setting. Qualifications

• Two (2) years of experience working on an interdisciplinary team.
• One (1) year of experience with frail or elderly population preferable.
• Current driver’s license and proof of auto insurance.

Responsibilities

• Plan, organize, and implement physical therapy services for participants.
• Provide high-quality care as a member of the interdisciplinary team.
• Document evaluations and treatment plans in participants’ medical records.

Skills

Bilingual and fluent in Hmong

Experience with elderly population

Knowledge of interdisciplinary teamwork

Education

Advanced degree in Physical Therapy

Current license to practice Physical Therapy
