Job summary

Qualifications

• DPT or PT Master’s degree from an accredited program.
• Current or eligible New York State PT license.
• Passion for geriatric rehabilitation and functional outcomes.
• Strong communication and teamwork skills.

Responsibilities

• Evaluate and treat patients in skilled nursing settings.
• Develop individualized treatment plans for orthopedic, neuro, cardiac, and deconditioning cases.
• Collaborate with interdisciplinary care team to achieve functional goals.
• Support patient return to highest level of independence.
