Job summary

Wurtland Nursing & Rehab is seeking a Physical Therapist to evaluate and treat residents in our skilled nursing facility. You will design individualized treatment plans to improve mobility, strength, and balance, working closely with an interdisciplinary team.

Join our compassionate team to support geriatric rehabilitation, educate residents and families, and supervise PT assistants while maintaining compliant documentation and quality care.

Qualifications

• Minimum 2 years of experience in physical therapy, preferably in a long-term care or geriatric setting.
• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist.

Responsibilities

• Conduct initial and ongoing PT evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, gait and balance training, mobility enhancement, and manual therapy.
• Educate residents and families on home exercises and safety strategies.
• Collaborate with nursing, OT, and speech therapy to ensure coordinated care.
• Oversee and supervise PT assistants; provide coaching and accountability.
• Maintain documentation per SNF, state, and federal guidelines.
