• Provide evidence-based rehab to seniors in post-acute and long-term care.
• Implement individualized treatment plans under supervision of a Physical Therapist.
• Deliver therapeutic exercise, gait and balance training, and functional mobility work.
• Educate patients and families to support goals and recovery.
• Maintain accurate, timely documentation of progress and outcomes.
