Job summary

Gurwin Healthcare System in Commack, NY, is seeking a per diem Physical Therapy Assistant to join our 36-acre campus team. Under the supervision of a Physical Therapist, you will deliver resident treatments, help achieve rehab goals, and maintain accurate records in accordance with facility policies.

This role supports our continuum of care across skilled rehabilitation, memory care, and long-term services, offering a dynamic environment and opportunities to grow in geriatric rehabilitation

Qualifications

• Associate degree from an approved PTA program or licensure.
• Two years of PTA experience preferred; 1 year geriatric experience preferred.

Responsibilities

• Provides treatments to residents as directed by the Physical Therapist.
• Modifies or adjusts treatment within the plan of care under supervision.
• Independently organizes a caseload reflecting productivity standards.
• Maintains records in accordance with guidelines.
• Participates in in-service and continuing education programs.
• Acts as Clinical Instructor, providing education to PTA students.
• Maintains knowledge of policies and procedures; suggests revisions.
• Participates in the Rehabilitation Department's Performance Improvement Program.
• Assists in planning or developing programs within Rehabilitation.
