Curana Health is seeking an experienced Psychiatric Nurse Practitioner to deliver high-quality mental health care across SNFs, LTCs, ALs and IL communities. You will work with a multidisciplinary team to provide comprehensive geriatric behavioral health services and ensure continuity of care for post-acute and long-term residents.

This role emphasizes compassionate, patient-centered care, ongoing collaboration with physicians, and support for families to improve outcomes while offering work-life

Qualifikationen

• This position requires PMHNP certification with ANCC.
• Active DEA license and state NP license are required.
• Experience with geriatric behavioral health in post-acute or long-term care settings is preferred.

Aufgaben

• Provide psychiatric care and rounding in SNF, LTC, AL, and IL settings.
• Collaborate with physicians, specialists, and facility staff to coordinate care.
• Assess residents, document histories, and manage mental health needs.
• Order and interpret tests, develop treatment plans for mental and physical health.
• Prescribe medications and provide injections as needed.
• Educate patients and families on meds and care plans.
• Collaborate with teams to address acute psychiatric and medical needs.
