Qualifications

• Board certified/eligible in Internal Medicine or Family Medicine.
• Fellowship in Geriatrics welcome, but not required.
• Active, non-probationary medical license in appropriate state.
• Collaborative, team-oriented approach to care.
• Deliver high-quality primary care addressing medical and social needs.

Responsibilities

• Provide comprehensive primary care to a Medicare-age population in a patient-centered environment.
• Work with a collaborative interdisciplinary care team and support staff.
