Founding Medical Director – Assisted Living Care

Compensation: $275,000–$325,000 + equity stake

We’re launching a next-generation post-acute care startup focused on transforming how healthcare is delivered in assisted living communities. As our Founding Medical Director, you will be Employee #1 on the clinical leadership team—helping shape care delivery, build clinical programs, and establish scalable systems to support growth across California and beyond.

This is a ground-floor opportunity with founder-level equity and a clear 5-year exit plan. We're looking for a bold, forward-thinking physician who sees the opportunity to lead innovation in elder care from the inside out.

What You’ll Lead

  • Clinical Oversight Across Communities: Serve as Medical Director for our partner assisted living communities, ensuring high-quality, compliant, and patient-centered care.
  • Model Development: Build and refine scalable protocols for chronic condition management, behavioral health integration, and transitional care.
  • Team Leadership: Recruit, mentor, and oversee a network of physicians and nurse practitioners across multiple locations.
  • Partnership Building: Collaborate with community leadership, nursing teams, and families to coordinate care and reduce hospitalizations.
  • Tech + Workflow Design: Influence the buildout of our EHR and care management tools to streamline clinical workflows.
  • Quality + Compliance: Monitor outcomes, lead QA efforts, and ensure compliance with all regulatory and licensing requirements.

What We’re Looking For

  • Board-certified MD or DO in Internal Medicine, Family Medicine, or Geriatrics
  • Minimum 3 years of clinical leadership or medical director experience (preferred in senior care or post-acute settings)
  • California medical license and DEA in good standing
  • Entrepreneurial mindset, with a passion for improving care for older adults
  • Strong communication, leadership, and systems-thinking abilities
  • $275,000–$325,000 annual base
  • Founder-level equity
