Eventus WholeHealth seeks an Advanced Practice Clinician (NP/PA) to deliver comprehensive primary care in skilled nursing and senior living communities. You will perform routine rounds, manage chronic diseases, and coordinate care with interdisciplinary teams within a value-based care model.

This role emphasizes preventive care, patient- and family-centered communication, and collaboration with facility partners.

Qualifications

• Current NP or PA licensure in the state of practice.
• Previous professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams
