Eventus WholeHealth seeks an Advanced Practice Clinician (NP or PA) to deliver comprehensive primary care within skilled nursing and senior living communities, including routine rounds and proactive chronic disease management.

You will collaborate with facility partners and interdisciplinary teams, leveraging AI-driven tools to enhance decision-making and support preventive, whole-person care, with a focus on quality outcomes and residents' experience.

Qualifications

• Current NP or PA licensure in the state of practice.
• Professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams.
